    By direction of the President, under the provisions of the act of Congress approved July 9, 1918 (Bul. No. 43, W.D., 1918), Cook Sylvester Smith (ASN: 1462185), United States Army, is cited for gallantry in action and a silver star may be placed upon the ribbon of the Victory Medals awarded him. Cook Smith distinguished himself by gallantry in action while serving with Company L, 140th Infantry Regiment, 35th Division, American Expeditionary Forces, in action from 26 September to 1 October 1918. During this time, Cook Smith worked over areas continually swept by machine gun and artillery fires, and although greatly fatigues, strove incessantly to bring back the wounded and on the return trips brought food and water to the troops in the front line. On 29 September he assisted in carrying his Commanding Officer, who was severely wounded at Exermont, to the dressing station at Chaudron Farm. Though knocked down several times, he doggedly remained at the stretcher until the successful culmination of his mission.
