The President of the United States of America, authorized by Act of Congress, July 9, 1918, takes pleasure in presenting the Army Distinguished Service Medal to Colonel (Dental Corps) Seibert D. Boak, United States Army, for exceptionally meritorious and distinguished services to the Government of the United States, in a duty of great responsibility during World War I. As Director of the Dental Section of the Army Sanitary School at Langres, France, from January to December 1918, Colonel Boak displayed organizing and training ability and accomplishments of the highest order in successfully directing the classification and training of dental officers for field service, thereby rendering services of great value to the American Expeditionary Forces.
