Private Eversole distinguished himself by gallantry in action while serving with the Medical Detachment, 148th Infantry Regiment, 37th Division, American Expeditionary Forces, in action near Heurne, Belgium, 2 – 4 November 1918. He further distinguished himself while crossing the Scheldt River during enemy bombardment in order to give first aid to the wounded.

Clay Eversole (ASN: 1543587), United States Army, for extraordinary heroism in action while serving with Medical Detachment, 148th Infantry Regiment (Attached), 37th Division, American Expeditionary Forces, near Cierges, France, 27 September 1918. During the advance of Company K, 148th Infantry, Private Eversole frequently exposed himself to great danger by carrying a number of wounded men through heavy machine-gun fire from an open field to a place of safety.
