The President of the United States of America, authorized by Act of Congress, July 9, 1918, takes pleasure in presenting the Distinguished Service Cross to First Sergeant Samuel E. Phillips (ASN: 1787095), United States Army, for extraordinary heroism in action while serving with Company B, 316th Infantry Regiment, 79th Division, American Expeditionary Forces, near Montfaucon, September 26 – 30, 1918. During the entire four days of action, Sergeant Phillips exposed himself to the dangers of artillery and machine-gun fire, assisting in every way possible to insure the success of the advance. He made repeated trips to the rear and cither urged his comrades forward or led them up to their positions. So strenuously did he labor during the entire action that, at the end of the fourth day, he was so exhausted from strain and shell shock that he was taken from the field.
