    (Citation Needed) – SYNOPSIS: The President of the United States of America, authorized by Act of Congress July 9, 1918, takes pleasure in presenting the Silver Star to Technician Fourth Grade Frank Forman (ASN: 34155359), United States Army, for gallantry in action while serving as a Medical Aidman with the 37th Infantry Division, in action against the enemy in the Pacific Theater of Operations, during World War II. The gallant actions and dedicated devotion to duty demonstrated by Technician Fourth Grade Forman, without regard for his own life, were in keeping with the highest traditions of military service and reflect great credit upon himself and the United States Army.

    The President of the United States of America, authorized by Act of Congress July 9, 1918, takes pleasure in presenting a Bronze Oak Leaf Cluster in lieu of a Second Award of the Silver Star to Technician Fourth Grade Frank Forman (ASN: 34155359), United States Army, for gallantry in action while serving with the 37th Infantry Division, in action against the enemy at Luzon, Philippine Islands, on 10, 11, and 20 February 1945. On three separate occasions Technician Fourth Grade Forman, a Medical Aidman with an infantry company, risked his life to aid wounded companions. Each time he braved intense enemy fire, and was forced to dangerously expose himself in order to reach the injured men. His outstanding courage, devotion to duty, and selflessness on these occasions saved the lives of his comrades, and were an inspiration to the men who witnessed the brave deeds. The gallant actions and dedicated devotion to duty demonstrated by Technician Fourth Grade Forman, without regard for his own life, were in keeping with the highest traditions of military service and reflect great credit upon himself and the United States Army.
