    Commander Amphibious Forces Pacific: Serial 00682 (September 18, 1945)

    The President of the United States of America takes pleasure in presenting the Silver Star to Lieutenant Carroll J. Boone, United States Naval Reserve, for conspicuous gallantry and intrepidity in action on 12 May 1945, as the Commanding Officer of a United States Ship during the assault on Okinawa. After his ship had shot down three suicide planes and a fourth crashed aboard with severe damage resulting, he, with outstanding leadership and courage, directed the removal of the wounded from the ship and when he gave the word to abandon the vessel due to a terrific blast, he personally searched the ship to make sure all of his men were safe. Through his profound devotion to duty, he proved to be an inspiration to all. His courage and conduct were in keeping with the highest traditions of the United States Naval Service.
