The President of the United States of America, authorized by Act of Congress, July 9, 1918, takes pleasure in presenting the Army Distinguished Service Medal to Lieutenant Colonel (Medical Corps) Samuel McCullagh, United States Army, for exceptionally meritorious and distinguished services to the Government of the United States, in a duty of great responsibility during World War I. As Chief of the Evacuation Branches, G-4, Paris Group, and G-4, 1st Army, Lieutenant Colonel McCullagh was charged with the supervision, coordination, and control of evacuation and hospitalization of the sick and wounded. By his great foresight, initiative, judgment, devotion to duty, and the helpful cooperation exercised by him, he played an important part in the successful treatment and evacuation of many thousands of sick and wounded under the most adverse conditions, performing this important and responsible duty with conspicuous ability, thereby contributing materially to the success of the American Expeditionary Forces.
