The President of the United States of America, authorized by Act of Congress, July 9, 1918, takes pleasure in presenting the Army Distinguished Service Medal to Lieutenant Colonel (Medical Corps) Dean D. Lewis, United States Army, for exceptionally meritorious and distinguished services to the Government of the United States, in a duty of great responsibility during World War I. As Chief of the Surgical Service of Evacuation Hospital No. 5 during the operations on the Marne, and the St. Mihiel, Meuse-Argonne, and Ypres-Lys offensives, by his tireless energy, organizing ability, and unusual surgical skill, Lieutenant Colonel Lewis successfully demonstrated that war wounds could be operated upon in large numbers in front-line hospitals with limited personnel, thus conserving many lives among combat troops, thereby rendering in a position of great responsibility, conspicuous service to the American Expeditionary Forces.
