The President of the United States of America, authorized by Act of Congress, 20 July 1942, takes pleasure in presenting the Soldier’s Medal to Warrant Officer (W-1) Jacob H. Stevens (ASN: W-3152141), United States Army, for heroism at the risk of life not involving conflict with an armed enemy while serving with the 179th Aviation Company, 1st Aviation Brigade, in the Republic of Vietnam on 9 October 1968. Chief Warrant Officer Stevens distinguished himself by exceptionally valorous actions while serving as aircraft commander of a CH-47 helicopter, loaded with ammunition, which crashed and burned from engine failure, trapping two men in the wreckage. Chief Warrant Officer Stevens unhesitatingly entered the burning ship to attempt a rescue of the men. Explosions from the ammunition were scattering fiery wreckage over a large area, and he was almost overcome by the intense heat and smoke. He located one man trapped underneath ammo boxes and twisted metal. Amidst the fire, he ripped out sheet metal, formers, and stringers in order to free the man, while explosions continually rocked the aircraft. Just as he pulled the man free, an artillery round exploded. Although the explosion stunned him, Chief Warrant Officer Stevens picked up the man and carried him from the fiery holocaust. Moments later, a series of rapid explosions turned the aircraft into a fireball. His actions were in keeping with the highest traditions of the military service and reflect great credit upon himself, his unit and the United States Army.
