The President of the United States of America, authorized by Act of Congress, July 2, 1926, takes pride in presenting the Soldier’s Medal (Posthumously) to Specialist Fourth Class Weyman Terry Cook (ASN: RA-12923755), United States Army, for heroism at the risk of life not involving conflict with an armed enemy while serving with the 114th Aviation Company, 1st Aviation Brigade, in the Republic of Vietnam, on 3 March 1969. Specialist Fourth Class Cook distinguished himself by exceptionally valorous actions while serving as crewchief of a UH-1D helicopter. The aircraft developed flight difficulties and crashed to the ground, bursting into flames upon impact. He managed to remove himself from the helicopter unharmed. As soon as he realized that the others were still trapped inside the burning aircraft, he rushed into the flames and pulled one of the survivors from the wreckage. As a result of his heroic action, Specialist Fourth Class Cook was severely burned and later succumbed to these fatal wounds. His actions were in keeping with the highest traditions of the military service and reflect great credit upon himself, his unit and the United States Army.
