Kostak (ASN: 1387719), United States Army, for extraordinary heroism in action while serving with Company G, 131st Infantry Regiment, 33d Division, American Expeditionary Forces, at Chipilly Ridge, France, 9 August 1918. Single-handed, Private Kostak, with great gallantry, attacked a machine-gun position, capturing two machine guns and seven prisoners.
