The President of the United States of America, authorized by Act of Congress, July 9, 1918, takes pleasure in presenting the Distinguished Service Cross to Major (Infantry) Herbert Clarence Fooks, United States Army, for extraordinary heroism in action while serving with 145th Infantry Regiment, 37th Division, American Expeditionary Forces, near Eyne, Belgium, 4 November 1918. Although severely wounded and his jaw shattered by a machine-gun bullet, Major Fooks refused to be evacuated, administered first aid himself, and continued to fearlessly direct his battalion during a strong counterattack. The personal example of this officer was a vital factor in the success of the operation.
