The President of the United States of America, in the name of Congress, takes pleasure in presenting the Medal of Honor to Captain (Infantry) Patrick Henry Pentzer, United States Army, for extraordinary heroism on 9 April 1865, while serving with Company C, 97th Illinois Infantry, in action at Fort Blakely, Alabama. Among the first to enter the enemy’s entrenchments, Captain Pentzer received the surrender of a Confederate general officer and his headquarters flag.
